At touch up I added a bit more to the left for areola size. She had #diepflapreconstruction by Dr. Nickolay Markov at WPAFB in Fairborn, Ohio. Her phase 2 included lifting the #diepflaps and creating areolas with surgically created #nipplereconstructions 🍨🍨 Her left nipple flattened so that side is completely #3d illusion and the right retained some projection to the nipple so it needed color tattooed.
I tattooed her last year and she came in recently to have more saturation and a darker tone applied.

I originally did these tattoos 5 years ago. However, when she had her implants switched for a larger size, her surgeon CUT RIGHT THROUGH the 3D tattoo on the right side. Most surgeons would not do that, however…it clearly needs to be discussed anytime you're planning a revision to prevent this. Most of the time, they can cut lateral to the tattoo OR use the IMF.
I originally tattooed this gal back in 2018. Since then, she's had a few surgeries at #ohiostateplasticsurgery that stretched her skin and placed considerably larger implants than she had before. This consequently stretched her tattoos, misplaced them a bit low and the surgeon...cut directly through one #3Dnippletattoo 👎, distorting the shape even more. 🤦♀️ I corrected this with a #tattooredo 👍
Take aways from this :
🧚♀️ try to make sure the tattoos are the final step of your #breastreconstruction 🎀if you're not happy with something that requires surgery, a tattoo may not help. It can help detract from some things but not everything.
🧚♀️ for future revisions, like implant switch out, discuss alternative incision choices with your surgeon to avoid situations like this! Most doctors (that I tattoo for anyway) will cut lateral to the tattoos on the original mastectomy incision or in the imf (inframammary fold). Always ask though 🤔!

The right nipple is completely flat and slightly higher than the left. So I was able to move it down. He left nipple has good projection. She will return later for top off.

#MASTECTOMYTATTOOS shown freshly done on bilateral immediate #diepflapreconstruction at #ohiostateplasticsurgery 🎀 She was referred to me by her surgical oncologist, Dr. Bridget Oppong . 💗 She had #radiationtherapy done to her right side after her #diepflap ➡️ so, why is that important to know❓You have to know as a tattoo artist that your canvas has been forever changed and essentially damaged by the radiation. This skin has visible changes looking darker (hence the tattoo looking darker on that side) and has telangiectasia present. It is firm, sitting higher and tighter on the chest. Most importantly, it's fragile and simply cannot be overworked. New inexperienced artists should not be working with this tissue. You 1st need years of machine experience with tattooing healthy skin... then proper hands on training experience. We will let this tissue #revealhowitwillheal and adjust color at touch up if needed and add more details.
Her radiated side on right healed a bit cooler and looked darker due to radiation skin changes present prior to tattooing. I balanced out at touch up adding color where needed.

She lost her implant on the radiated side which required #diepflap reconstruction. She also lost her reconstruction on her non-radiated side and was so damaged she required a #latissimusdorsi flap. We had a discussion regarding her very thin skin and scar on her right side prior to tattooing and realistic expectations. This is her result after her first session. Notice the poor ink take on the lat flap scar. This tissue is so thin and damaged. Using the same concentration ink on both sides you can see the struggle with this tissue. At touch up I used full strength ink and was very careful with implantation.
